VA Benefits Request Form (BRF)
This form is required by Veterans Services anytime you are requesting VA benefits for a particular semester.

 

Change in Benefits Form
If you are changing the number of hours (credits) you are taking for a particular semester and have already filled out a Benefits Request Form, this is the form to use to notify us of your change.

 

USF Veterans Services Forms
Depending upon your situation, one or more of these forms might be required as part of your VA certification process. Most of these forms need to be signed by an authorized college advisor. Check the first link, "Authorized Signatures for VA Certification Forms" for the appropriate advisor.
